I am voluntarily participating in an online yoga class (collectively referred to as, the “Activity”) offered by Paige Phelps, Certified Yoga Instructor 500 RYT | 200 RYT

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the above mentioned Activity. I acknowledge that I have no medical condition that would prevent my participation in the Activity. I am aware that participating in the Activity could result in physical injury and may aggravate pre-existing injuries. I understand my physical limitations and I am sufficiently self-aware to stop or modify my participation as necessary for my own safety.

I will follow all instructions given to me by the Instructor as to when, where, & how to perform or not perform yoga activities. Any deviation by me from such instructions shall be at my own risk. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the Activity, including those which may result from the actions of the Instructor.
